Celebrate a nurse today!!!

The CNB Award Nomination forms are now open. Use this link CNB Award Nomination Form to complete the award nomination and celebrate a nurse today.  An example form is attached.  Please use this for reference and use the link for the nomination submissions. 

Guidelines:

• The nomination review process is a blind review. Do not use names in the context of the application. Use pronouns or 'nominee'.
• Resumes are not accepted for review. 
• Previous Celebrate Nursing Honoree recipients and current BRDNA Board Members are not eligible for this award.
• Applications not meeting the above guidelines and not addressing the appropriate number of criteria WILL NOT be reviewed. 

Instructions for completion of the form:

• Identify HOW the nominee has contributed to the community, nursing, and health care using the criteria below.  
• Criteria 1 MUST be addressed, AND choose two criteria from 2 through 6. 
• Identify the criteria you are selecting and include specific examples of how the nominee has met them.
